The Efficacy Gap Is Real

Oral Wegovy delivers the same approximate weight loss as injectable Wegovy — about 15% of body weight. Foundayo delivers approximately 11-12%. That 3-4 percentage point difference matters: for a 220-pound person, it's roughly 7-9 additional pounds lost over the course of treatment.

However, Foundayo's ATTAIN-MAINTAIN trial showed that patients who switched from injectable GLP-1s to orforglipron maintained most of their prior weight loss — meaning starting injectable and transitioning to oral is a viable strategy.

The Convenience Gap Is Also Real

Oral Wegovy's fasting requirement is genuinely burdensome. You must take it first thing in the morning, on an empty stomach, with a small sip of water, and then wait at least 30 minutes before eating, drinking, or taking other medications. In practice, this means restructuring your morning routine around the pill. For people with early work schedules, multiple morning medications, or inconsistent routines, adherence drops.

Foundayo has none of these restrictions. Take it whenever, with whatever. This simplicity is clinically meaningful — a medication you actually take consistently outperforms a superior medication you skip.

The Cost Picture

Oral Wegovy carries the same list price as injectable Wegovy (~$1,350/month). Foundayo starts at $149/month self-pay. For patients without insurance coverage, this makes Foundayo dramatically more accessible. But compounded injectable semaglutide — the same active ingredient as oral Wegovy — remains the most affordable option.
